Waivers must be completed, signed and returned to Pleasant Hill Recreation & Park District prior to participation in a class or activity. Some waivers will be circulated for signature at the first meeting. If required in advance, please print form, complete it (please be sure it is signed) and mail or deliver the original to our District Office. Emailed, faxed or electronic copies of the waivers will not be accepted.
